Request to Add a Guest to Canvas Site

Please complete and submit this Form to ask for approval from your division dean to have guests added from your live course sites in Canvas each quarter.

In accordance with student privacy guidelines (FERPA), only enrolled students and assigned faculty should have access to live course sites unless there is a compelling reason such as:

Co-teacher
Course reviewer

To give guest access to non-employees or non-enrolled students, please request a Backup site for your live site where you can import the content of your live site and add People yourself. Once we set up the Backup site, you will be able to

2. What is your email address? This question requires a valid email address.

3. What is the first and last name of the person you want added to your Canvas site as a guest?

4. What is the email address of the person who you want added to your Canvas site?

5. What is the college-wide ID number (CWID) of the person who you want added to your Canvas site? If your guest does not have a CWID, please explain why not.

6. What is your reason to add this guest to your live Canvas site?

To find the URL, go to your course site, select the webpage address in your browser, and then copy/paste that web address into this form.
